What is Chronic Care Education?

Chronic Care Education is a health education platform dedicated to helping individuals, families, and caregivers understand and manage chronic health conditions with confidence. Through expert-led content, and practical tools, we transform complex medical information into actionable knowledge that empowers people to take control of their health.

Whether you're navigating diabetes, heart disease, high blood pressure, or autoimmune disorders, we believe that informed care is better care. Our mission is to bridge the gap between medical advice and everyday life by making health literacy simple, inclusive, and engaging.

Our Why

With 15 years of school health experience, I’ve noticed there was a deficit of knowledge related to the care of students with diabetes in the school setting. In July 2025 I answered a call to facilitate a workshop to bridge the knowledge gap for nurses. Providing this specialized care for students, parents and school staff has become my mission.

Based on the need to close knowledge gaps, in 2017 I became a Certified Diabetes Educator (CDE) also known as, Certified Diabetes Care and Education Specialist (CDCES).
